Connection Card

It is our desire to connect with and minister to every person who attends Harvest. An important step in this process is ensuring we have accurate information to be able to communicate with you. It is also helpful to understand your needs and interests.

This card is intended to gather information that will allow us to communicate with you and get the right person or team connected with you for any particular requests. If you have a prayer request to share, please fill out a "Prayer Request" form. If you would like to volunteer to serve in different areas of the church, please fill out a "Volunteer to Serve" form.
